Recurrent Left Ventricular Apical Pseudoaneurysm at the Site of Previous Patch Repair; a Case Report

: Our case was a 70-year-old male with past history of coronary artery bypass graft surgery and true aneurysmectomy seven years ago who presented chest pain dyspnea. Echocardiography demonstrated scarred aneurysmal apex (6.15 cm × 2.19 cm) small rupture just at most protruding part it, to from flow across this site resulted in slow oozing blood into pericardial sac mobile clot apex, formation extended both apical parts distorted apicoseptal geometry without communication right ventricular cavity. Cardiac MRI (CMR) showed the following data: large pseudoaneurysm ruptured patch (depth: 46 mm, width: 60 mm), narrow orifice (5 septated left outpouching surrounded by layers included clots different sizes ages. He underwent successful surgical operation. In case, CMR provides excellent images essential for patient management.
